(1) Application Form;
(2) Photocopy of passport;
(3) 4 recent photos (2 inches size);
(4)Senior high school diploma(notarized copy in Chinese/English);
(5) Transcript for all the courses in senior high school (notarized copy in Chinese/English);
(6) Police Clearance Certificate issued by the National Police Authorities of home country of origin or residence;
(7) Photocopy of Physical Examination Form for Foreigners. Please select the appropriate time to take medical examination due to the 6-months validity of the medical results;
(8) Application Fee: 600/person.
